Community Healthcare Course: Addressing Disparities

As a Community Healthcare Worker, you know that populations accessing services at federally qualified health centers face great challenges to optimal health, leading to a 40% higher risk of death after adjusting for age in some cities.

This is why the American Heart Association, with input from the Institute for Public Health Innovation and support from the Bristol-Myers Squibb Foundation has developed a free resource covering the basics of atrial fibrillation, venous thromboembolism and stroke—conditions disproportionately affecting populations that access services at federally qualified health centers.

Visit bit.ly/ahachw-partofcareteam to learn more about addressing disparities in cardiovascular care.
